facility stated traveler did not complete a rounding sheet and that there was a patient complaint. traveler states the patient complaint is not correct and they followed hospital protocol. traveler states the facility had low census. traveler also states the facility had the wrong end date on their copy of the contract so they sent a copy of their contract to the manager and they were canceled the next day.
